Important Information

  • All new prescription requests require a consultation. To request a repeat prescription, this can be done via our online portal by clicking HERE.

  • All new referral requests to a Specialist require a consultation. To request a repeat referral to replace a previously expired referral, this can be done via our online portal by clicking HERE.

  • If you require forms to be completed by your doctor, an appointment must be made to discuss the requirements. A private fee will be payable for paperwork requests.

  • All results e.g. blood tests, scans, x-rays, excisions etc. require a consultation (telehealth or face to face) to discuss these with your GP, unless your doctor has instructed differently. This ensures clear explanation and ongoing advice/management.

  • All medicals and procedures require a long consultation, please advise reception staff when booking.

  • A Health Assessment booking must be made by phoning the practice.

  • If you do not have a current valid Medicare card in your possession or on our practice records, you will be charged the full fee payable on the day. We are unable to call Medicare on your behalf.

Cancellation Policy
If you are unable to attend your appointment, please inform our reception at least two hours before your scheduled appointment time so it may be offered to another patient. 

If you fail to cancel your appointment within this time frame and/or do not attend, we may need to charge a non-attendance fee.


Medicals
Pre-employment medical assessments and other medical assessments are privately billed. We offer bulk billed health assessments for patients:

  • Aged between 45-49 years (Chronic Health Assessment)

  • 75 years and older (Chronic Health Assessment)

  • Aged 40-49 years at risk of developing chronic diseases such as Type 2 diabetes as determined by the Australian Type 2 Diabetes Risk Assessment Tool (AUSDRISK) 

These services are Medicare rebated every 12 months. If you would like more information about these services please call our practice and speak with our reception team.
