We welcome new patients to Mosman Park Family Practice.

For all new patients, please complete a New Patient Registration Form prior to arriving at the practice.

We believe quality health care cannot be rushed, and that it takes time to properly explore a person’s health with a big picture approach. Therefore, our default is to book new patients for 20 minute consultations.

If this is your first visit to the medical practice, please bring your Medicare card and a photo ID (driver’s licence/student card). If you cannot complete the form prior, please arrive 10 minutes ahead of your scheduled appointment time to fill in necessary paperwork.

At all times we will endeavour to offer you an appointment with your regular doctor. There may be occasions when this is not possible due to emergencies, holidays, etc. and we will try to accommodate you as best we can.
