COVID-19 Vaccination Information

We are pleased to continue to offer Telehealth services to our patients who are concerned about COVID-19 risks. If you have a cough, fever, sore throat or have travelled recently or have been in contact with COVID-19 please call us so that we can arrange the most suitable care for you.

Mosman Park Family Practice is an approved COVID-19 vaccine provider.

We are currently administering BA 4-5 Booster vaccine 12 years +.

MAKING AN APPOINTMENT

  • All vaccine appointments must be made online through HotDoc here.

  • Patients who have not been seen at our practice before will need to bring their Medicare card

  • The COVID-19 vaccine is free

Preparing for your appointment

After you have made your booking online as a new or existing patient, and once you have confirmed your appointment reminder, you will receive an email request to complete a consent form. Please complete and submit this prior to attending your appointment. Complete your online consent form HERE.

Before your appointment, you should also make sure your details are up to date with Medicare.

Please do not come to your appointment if:

  • You are feeling unwell with fever, cough, runny nose or other symptoms that could be from COVID-19

  • You are waiting for COVID-19 test results, or have tested positive for COVID-19

  • You are a close contact of someone with COVID-19, or
